6th Grade Field Trip to Victoria, BC
May 6, 2004

Dear Parents:

Our 6th grade class trip to Victoria, BC, which supports our studies of Canada, will happen on May 6th. Your child will need several items for this trip:
1. A copy of his/her birth certificate, social security card, or a passport. The office staff has been collecting this information over the past few weeks. Many of you have already provided this information.
2. A check for ferry transportation and the Royal BC Museum entrance fee made out to Chimacum Middle School---
•The ferry- if your child is 12 years or older on May 6, 2004, the cost is $18.00. If your child is 11 years old the cost is $9.00 US
•The museum is $4.75 for students and $6.50 for adult chaperones

So, the total for 12 yrs. old and over is $22.75
the total for 11 and younger is $13.75
the adult total is $24.50

3. Your signature giving permission for your child to travel to Canada, on the attached sheet.

ALL THE ABOVE ARE DUE BY MONDAY, May 3rd!!!

Behavior
This is a huge concern. We cannot take the risks on student behavior in Canada. Students need to prove they are responsible and trustworthy here at Chimacum so we can depend on them in Victoria. This includes students that are missing many assignments as well. Teachers have kept track of behavior issues with students and will contact parents with any concerns.

Chaperones
Many parents have already asked about helping out with this trip. Please remember you will be responsible for a group of students the entire day, including the ferry trip. This is a high energy job, and we need your good energy and effort to help set an example for the students. Chaperones will have to pay $24.50 for ferry and museum fees. You can include this payment with your son/daughter’s costs.

I would like to attend as a chaperone. _________________________.
